This is an NHS scheme, your local Jobcentre Plus Office can confirm entitlement to the benefit.
What is it?
• Help with the cost of your fares or petrol when you go into hospital for NHS treatment and when you come out of hospital
• Help with the cost of your fares or petrol when you go to and from hospital as an out-patient for NHS treatment
Can I get it?
If you get:
Income Support
or
Income-based Jobseeker's Allowance
or
Working Tax Credit or Child Tax Credit under certain circumstances
Then you can get help with travel costs to hospital.
If not, you may be able to get help with travel costs through the NHS Low Income Scheme.
Notes
If you need someone to travel with you, you may also be able to get help with the cost of their fares.
How to claim
If you get Income Support or income-based Jobseeker's Allowance, tell the hospital that you want to claim and show them proof that you are entitled to benefit.
If you want to claim under the NHS Low Income Scheme, get form HC1 Claim for help with health costs from your social security office or an NHS hospital.
If you want more information, get leaflet HC11 "Are you entitled to help with health costs?" from your social security office, post office or NHS hospital.
